For our customers with insurance, please remember the importance of paying the co-pay amount that your insurance specifies. This amount is due at each office visit, and its prompt payment helps to keep the cost of care affordable for all of our patients, and is important to our ongoing ability to provide vital health care services.
If you do not have insurance coverage and require medical care, please contact our business office. Valley Health Care offers a variety of payment plans and programs to help our patients. We Accept Health Insurance and Credit Cards
Valley Health Care accepts most forms of payment. A wide variety of health insurance plans, workers’ compensation, credit cards (including Visa, MasterCard, Discover, and American Express), and debit cards can be used as payment, as well as cash and checks (with proper state-issued identification). We offer our Sliding Fee Discount Program for uninsured and underinsured local residents without insurance coverage who meet specific income criteria, based on household size. For more information about the Sliding Fee Discount Program or to apply, please ask the receptionist or one of our front office staff for assistance. Please remember to bring to following with you to each visit:
- All active insurance and medical cards
- Your valid WV state drivers license or photo ID card
- A list of your current medications (or bring the bottles in with you)
Sliding Fee Scale at Valley Health Care
Valley Health Care offers an extensive variety of services and are pleased to provide most of our services through the Sliding Fee Scale. We provide the Sliding Fee Scale payment option for our income-eligible patients based on annual household income. Income guidelines apply. Please be prepared to provide the following acceptable income verification during the application process:
Gross income (ALL income from ALL sources prior to taxes) for all immediate family members in your household. Acceptable forms of income verification are:
- Current Federal Income Tax Return
- Two Most Recent Pay Stubs (One Month of Income)
- Social Security Benefits Letter
