Appointment and Check-In Procedures

Please check-in with the receptionist immediately upon arrival. To help us best serve you and your family, please plan to arrive 20 minutes prior to your scheduled appointment time. If you anticipate a delay, please notify us in advance.

If you must reschedule an appointment for any reason, please call and let us know at least 24 hours before your scheduled visit. Patients who repeatedly cancel appointments without providing a 24-hour minimum notice, or who fail to arrive, may not be allowed to schedule future appointments.

At Valley Health Care, we strive to provide each patient with personalized care and attention, and sincerely appreciate your courtesy and patience. Due to situations beyond our control, such as a provider’s emergency or illness it is sometimes necessary for us to reschedule appointments on short notice. We make every reasonable effort to accommodate our patients’ needs.

What to Bring With You to Your Appointment

If you are a first time patient please pre-register. To find out more about pre-registration, contact the office of your appointment. If you did not pre-register you must arrive approximately 30 minutes prior to your scheduled appointment. This will allow sufficient time for us to collect your medical history, insurance information, photo identification, and allow time for you to complete the forms required for your treatment.

Please bring the following items with you to your first, and each subsequent, appointment:

If you require a prescription refill, or school/work excuse, please notify us immediately upon your arrival. We will forward your request for you. Bring any paperwork or forms that you would like your doctor to complete. Please note that a fee may apply for form completion, and plan to allow sufficient time for the form to be completed and returned to you.
