Valley Health Care offers our Sliding Fee Discount for uninsured and underinsured residents in West Virginia. If you do not have medical insurance, or if you have high copays and/or deductibles consider applying for this valuable medical discount program. Our Sliding Fee Discount Program is designed to meet the health and vision care needs of individuals who meet specific income guidelines. Fees are based on a sliding scale that is based upon both income and the number of people in your household. Please note that federal guidelines are subject to change. Valley Health Care is a Federally Qualified Health Center (FQHC) and a National Health Service Corps (NHSC)-approved facility serving Mill Creek, Elkins, and surrounding areas of West Virginia. Sliding Fee Scale at Valley Health Care

Valley Health Care offers an extensive variety of services and are pleased to provide most of our services through the Sliding Fee Scale. We provide the Sliding Fee Scale payment option for our income-eligible patients based on annual household income. Income guidelines apply. Please be prepared to provide the following acceptable income verification during the application process:

Gross income (ALL income from ALL sources prior to taxes) for all immediate family members in your household. Acceptable forms of income verification are:

  • Current Federal Income Tax Return
  • SSA-1099
  • Two Most Recent Pay Stubs (One Month of Income)
  • Social Security Benefits Letter
