We accept a variety of insurance plans and offer our VHC Care Program for those without insurance. Contact our Business Office to discuss payment arrangements. For each visit we ask that you bring the following items:
Your insurance or medical cards
Current medication bottles and/or a list of current medications
Current driver’s license or photo ID
We will accept your Medicaid medical card as payment in full for the services that are covered. You are responsible for paying for any services that are not covered by Medicaid.
Medicare assignment means that we bill Medicare directly for all covered services and accept the approved amount. You are responsible for a 20% co-payment for all non-preventative services. Valley Health Care is a Federally Qualified Health Center (FQHC), so there is no annual deductible. You are responsible for paying for any services that are not covered by Medicare.
We will bill your workers’ compensation plan for any covered injury under its regulations.
We will bill your insurance company for services that we provide. We ask that you provide all billing information at every visit. Co-pays are due at the time of service. You are responsible for paying for any services that are not covered by your insurance plan.
MANAGED CARE PLANS
We contract with various plans and continue to negotiate with additional groups. Please ask a receptionist for a list of plans in which we participate.
